By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, that any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal.
In consideration of my desire to serve as a volunteer, I hereby assume all responsibility for any and all risk of property damage or bodily injury that I may sustain while participating in any voluntary position with Seminary Covenant Community and I hold harmless Seminary Covenant Community, Inc and Brevard Public Schools from any liability.

I understand that it is my responsibility to become a registered volunteer and pay any costs associated to get and maintain this status. If at any time my volunteer status is revoked with BPS, I will not be able to perform my voluntary duties assigned and it is my responsibility to communicate this status change with the Seminary Covenant Community leadership.
